Ben Edward Leff, M.D.
Ben E. Leff, M.D. earned his medical degree at the University of Pittsburgh, in Pennsylvania and completed his residency in Otolaryngology at the Cleveland Clinic Foundation in Cleveland, Ohio. He is board certified in Otolaryngology. Dr. Leff is a Fellow of the American Academy of Otolaryngology and a member of the Arizona Society of Otolaryngology. Dr. Leff served as Department of Surgery Chairman at John C. Lincoln Hospital, North Mountain from 2010 through 2014 and has been voted Phoenix Magazines’ Top Doctor in 2008, 2010 and 2012. Outside of the office, Dr. Leff enjoys hiking and spending time with his wife and two children.
Deven S. Gujrathi, M.D.
Deven S. Gujrathi, M.D., earned his medical degree at Queen’s University, School of Medicine Kingston, Ontario, Canada and completed his residency in Otolaryngology at the University of Manitoba in Winnipeg, Canada. He is board certified in Otolaryngology. Dr. Gujrathi is a Fellow of the Royal College of Surgeons in Otolaryngology and Member of the Arizona Society of Otolaryngology. Dr. Gujrathi is a staff surgeon at John C. Lincoln Hospital and Phoenix Baptist Hospital and a consulting Otolaryngologist for Rehabilitation Hospital of Rhode Island. He specializes in treating diseases of the ears, nose, throat, neck, including facial plastic surgery, facial and neck trauma, otolaryngic allergy, sleep apnea/snoring and voice and swallowing disorders for patients of all ages. Outside of the office, Dr. Gujrathi enjoys hiking, rafting, skiing and spending time with his wife and 3 children.
Kevin Kristalovich, Au.D., CCC-A
Dr. Kristalovich earned a master’s degree in audiology from Idaho State University at Pocatello and a Doctor of Audiology from A.T. Still University at Mesa, Arizona. He interned at the University of Colorado Health Sciences Center at Denver before completing a fellowship at HEARx Ltd in West Palm Beach, Florida. Dr. Kristalovich brings more than 20 years of clinical experience in the field of audiology. He provides comprehensive diagnostic evaluations and specializes in hearing aid selection and fitting, aural rehabilitation and balance issues. During his off-hours, you’ll find Dr. Kristalovich enjoying anything related to his interests in professional hockey and car clubs.
